The Ambulatory RN Care Manager provides care management services and community resource coordination to patients and their families who utilize inpatient and outpatient medical services. This position is part of a centralized care coordination model for the Innovative Health Alliance of New York (IHANY), serving attributed lives, in both their primary and specialty practices. This position will be based out of the St Peter’s Health Partners Medical Associates care coordination department. BSN and 5 yrs of previous experience in care management required.
Pay Range: $31.50-44.95
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
Job Type: Full-time
Pay: $65,520.00 - $93,496.00 per year
