Key Responsibilities
- Provide management and delivery of patient care plans
- Support patients in Wyomissing, PA and surrounding areas
- Consistently meet critical deadlines for charting
- Self-manage time and schedule
- Full-time Monday- Friday 8a-5p. Currently their on-call rotation is only 1 weekend every 5-7 weeks depending on staffing. When it is their scheduled weekend, they are on-call Friday from 5pm-8am, Saturday & Sunday from 8pm-8am.
- Occasional local travel
