Fall River Group, Inc is Hiring a Social Worker Near Raynham, MA
Duties and Responsibilities:
- Provide case management services
- Develop and lead groups for the participant
- Assist the program coordinator in referral, admission and discharge process
- Write progress notes quarterly
- Inform participants and their families/caregivers of available community services, and make referrals as indications warrant
- Attend monthly supervisor meetings with the Program Coordinator (Bi-weekly with LICSW if not licensed)
- Assist, as necessary, in the delivery of other required program services
- Participate in the bi-annual or quarterly review of each participant’s care plan as well as assisting with the development of care plans
- Assist Program Coordinator with the implementation of policies and procedures
- Assist Program Coordinator with record reviews, documentation, compliance, and implementation of policy and procedure
- Ability to provide services outlined in this job description at any of the three Adult Day Health sites as assigned by the Program Coordinator
- Respond to reasonable expectations of the Agency and/or supervisor
- Bachelors Degree required
- At least One (1) year of recent social work experience working with disabled/elderly adults in a professional capacity is required.
