The Hampton Roads Community Health Center is looking for a Registered Dietician to collaborate with the interdisciplinary healthcare team to improve the health and physical conditions of the patient’s nutritional needs including serving as a subject matter expert in diabetes management. Reports to the Chief Medical Officer
Analyzes patients’ health status, goals, and dietary restrictions, tracking dietary intake and progress and educating patients on how to maintain a balanced diet; creates or gathers all educational materials/handouts used to educate patients; and provides counseling, nutritional assessments, prescriptions, and evaluation of nutrition-related lab parameters to improve results and maximize outcomes.
Schedules appointments for patients attending the sessions for individual follow- up appointments; assists with referrals, obtaining lab results, medication refills, medical records, and scanning/faxing records; obtains and evaluates nutrition histories of the patients regarding food habits; and assesses patient’s knowledge of diet and kidney disease and provides appropriate detailed education to the patient and their family regarding nutritional status.
Conducts Centering Diabetes class sessions twice a month, each session will be 2 hours; conducts comprehensive nutrition assessments using the Nutrition Care process (NCP) model including nutrition focused physical exam (NFPE); participates in review of the class and outcomes every 6 months throughout the duration of the program; and completes bi-annual DSME Program report and convey results to the Risk Management committee and Executive leadership.
Develops and implements individualized therapeutic plans for specialized needs; monitors nutritional status, laboratory values, medications, dialysis kinetics, adherence and response to diet and/or other nutrition therapy; documents all clinical actions in the electronic medical record and review the chart for completeness, test results and billing; and coordinates the education of medical staff, nursing staff, and families on the medical nutrition therapy for outpatient plans.
Performs administrative support work such as word processing, creating spreadsheets, data entry and retrieval; orders, monitors and distributes program supplies/resources; and other information to ensure accuracy and conformance to established procedures and policies; and counsels patients regarding patient responsibilities and rights; adheres to patient confidentiality guidelines.
Performs other duties as assigned.
Bachelor's degree in Nutrition or related field and at least three (3) years of dietetic experience in a clinical setting or any equivalent combination of education and experience. Must have completed a state-approved certification program for a Registered Dietitian or Certified Diabetic Educator. Must be certified in Basic Life Support techniques.
