Job DescriptionThe Case Manager will be responsible for providing case management services to clients in need. This includes assessing client needs, developing and implementing care plans, coordinating services with other providers, and monitoring client progress. The Case Manager will work closely with clients, families, and other service providers to ensure that clients receive the best possible care.
Responsibilities
- Conduct initial assessments of client needs
- Develop and implement care plans
- Coordinate services with other providers
- Monitor client progress and adjust care plans as needed
- Provide ongoing support and guidance to clients and families
- Maintain accurate and up-to-date client records
- Participate in team meetings and case conferences
- Collaborate with other service providers to ensure comprehensive care
- Bachelor's degree in social work, psychology, or related field
- Experience in case management or related field
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong organizational and time management skills
- Knowledge of community resources and services
- Ability to maintain confidentiality and adhere to ethical standards
