Responsibilities

Cedar Hills Hospital is seeking a QDischarge Plannerwith an background in behavioral health to join our team and support patients!

How you will make an impact:

The Discharge Planner advocates for patient access to services during treatment team meetings, helping to support and coordinate ongoing patient care and aftercare needs. Additional responsibilities include:

  • Promoting effective use of resources for patients
  • Advocating that the patient is placed in the appropriate level of care and program
  • Maintaining documentation related to case management activities
  • Oversight of continued communication of treatment progress, discharge plans, and time of discharge to outside parties
  • Facilitating the transfer of required documents to the outside agents

Qualifications

Qualifications:

  • Bachelors from an accredited college or university in social work, mental health, psychology, nursing or related field preferred.
  • Experience in a healthcare setting or managed care
  • Patient assessment related to discharge planning, treatment planning, and/or case management experience preferred
  • May be required to work occasional overtime and flexible hours
