Overview:
We are seeking a dedicated Licensed Practical Nurse to join our healthcare team. The ideal candidate will provide compassionate care to patients and support our medical staff in delivering high-quality healthcare services.
Duties:
- Perform patient assessments and provide basic medical care under the supervision of a Registered Nurse or Physician.
- Administer medications and treatments as prescribed by healthcare providers.
- Assist in developing and implementing patient care plans.
- Monitor and report changes in patient symptoms or behavior.
- Educate patients and their families on healthcare procedures and treatment plans.
- Collaborate with the healthcare team to ensure optimal patient outcomes.
- Utilize electronic health records systems such as Epic for accurate documentation.
Requirements:
- Completion of an accredited Licensed Practical Nurse program.
- Current LPN license in the state of practice.
- Strong knowledge of discharge planning, catheter care, pain management, memory care, and dialysis procedures.
- Proficiency in conducting patient assessments and providing appropriate care.
- Experience with suture removal, Durable Medical Equipment (DME), and working in assisted living facilities is preferred.
- Excellent communication skills and the ability to work effectively in a team environment.
