Put your talents to use in a culture of care where you can make the world a better place.Position SummaryA Case Manager provides continuity of care, with the goal of permanent placement, for children through a case management model that includes developing, expanding, accessing and linking resources in the community to the needs of the child throughout the child’s experience in the system, while documenting progress.
What You Will Do
  • Conduct assessments, plans, links services, monitors progress, and advocates for assigned children and families from intake to discharge.
  • Develop comprehensive service/case plans including:
  • Realistic permanency goal achievable within 12 months.
  • Measurable objectives linked to the permanency goal.
  • Specific task assignments with timelines.
  • Ensure compliance with legal requirements under Chapter 39.
  • Review and updates service/case plans monthly or as needed.
  • Ensure progress towards permanency or makes timely modifications.
  • Prepare and disseminates monthly progress reports to stakeholders, including parents.
  • Update service/case plans when necessary, adjustments are needed.
  • Identify and monitors children and families at risk.
  • Advise supervisor of their status.
RequirementsWHAT YOU NEED:
  • BA/BSW or MA/MSW
  • experience in Foster Care/Adoption is preferred, but not required
  • 24 months previous work experience
