Job Summary:
The LPN provides direct supervision over the quality care and assures agency compliance with State requirements for service delivery and caregiver training.
Responsibilities:
- Perform In-Home client assessments, report finding and make recommendations for changes in the plan of care.
- Supervise client care and recommend for improving the client's services and to obtain doctors orders for each client they service.
- Assist in training of Aides and Homemakers.
- Provide care to client as authorized.
- Supervise required clients on site programs and assure appropriate documentation.
- Supervise Advance Personal Care Program.
- Must have knowledge of geriatric patient care and be able to access support system to meet client needs.
- Must have knowledge to solve problems and work closely with client, family, caregiver and support agencies in maintaining quality care.
- Must have reporting skills to meet state agency requirements.
- Must be organized.
Qualifications:
- Must be able to get in and out of vehicle.
- Must be able to get in and out of consumers home.
- Must be able to drive long distance between consumers.
- Must be able to drive in inclement weather where permitted.
- Must be able to tolerate consumer's pets, smoking, and personal hygiene.
- Must be able to lift at least 25 lbs.
- Must be able to sit and type.
- Must be able to file paper work that requires lifting, standing, reaching and bending.
- Must be able to travel out of state for meetings which may include cars, buses, trains, and planes.
- Must have a clear background check or have an approved good cause waiver with the Family Care Registry. Name must not appear on the EDL.
Job Type: Part-time
Pay: $16.00 - $18.00 per hour
