Mt. San Antonio Gardens is hiring CNAs for Harrison Villa, a small home for 10 residents who require an enhanced level of individualized care, support with the Activities of Daily Living, and programming for individuals with distinct healthcare needs beyond the level of care regularly provided in Assisted Living.

We are hiring passionate CNAs to work in a homecare setting responsible for assuring safe and compassionate delivery of health care and related services to residents in Harrison Villa during assigned shift. Our model of care offers an alternative model to traditional care in that it focuses in creating an authentic, caring home environment where residents can thrive and grow even as their needs increase, providing person-centered care through non-institutional eldercare environment and building strong relationships with each resident. FT AM Shift 6:45-3:15 p.m. - 5 days/week

PT AM Shift 6:45-3:15 p.m. - 4 days/week

Responsibilities:

• Recognize and respond to the needs of elders
• Promote autonomy of elders in decision-making
• Maintain confidentiality and privacy of elder care, procedures and documentation
• Communicate with co-workers at all levels to meet the needs of elders
• Meet with the CST as scheduled or needed to discuss elders’ needs and progress in identifying and correcting problems
• Observe, monitor and report to the CST any observed change or suspected change in elder condition
• Document observed data per organization policy, report changes in the elder’s condition to the CST
• Assist elders with personal care functions, including: Dental and mouth care, bathing, dressing and grooming, hair care, nail care, shaving, bowel and bladder care, other personal and hygiene care
• Assist with and/or perform restorative and rehabilitative procedures as outlined and assigned by thecare plan and with appropriate training
• Measure and record vital signs such as temperature, pulse, respirations, weight, height and pain
• Assist moving, positioning and transporting elders into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
• Maintain a lift-free environment by utilizing the lift systems provided in the houses
• Provide range of motion exercises as outlined and assigned by the care plan and with appropriate training
• Prepare and serve meals, between meal, bedtime and other nutritional snacks per the posted menu and the stated preferences of the elder
• Observe standard precautions per facility policy
• Keep all areas of The Green House® home clean, performing routine light housekeeping duties
• Make occupied and unoccupied beds and change bed linens
• Launder clothing, linen and other articles
• Follow established safety precautions and observe, monitor and intervene in or report to the Guide or RN any unsafe conditions per facility policy
• For those identified elders, assure that a call bracelet is being worn by the elder at all times and answered
• Report accidents and incidents when they occur per facility policy
• Report any resident abuse or suspected abuse to the Guide, DONS, RN and/or the Administrator immediately
• Follow established smoking regulations and report violations
• Attend and participate in orientation programs, on-going training and educational classes
• Attend and participate in Care Planning Meetings and collaborate with the CST to schedule the Care Planning Meetings
• Seek counsel and assistance with problem-solving from the Sage
• Maintain a safe environment for all Residents.
• Perform other related duties as required by the elder
