Service Coordinator for Senior/Disabled Congregate Living Complex

For a 32 unit Senior/Disabled apartment complex in Pine City, NY. Qualified applicants must have the proven ability to recognize and assess the needs of individual residents and be able to complete referrals to outside agencies that help the resident continue to safely live an independent lifestyle for as long as possible. Position includes the planning and implementation of group educational, wellness and social activities along with the oversight of a congregate meal program. Prior work experience with the elderly and/or disabled in a human services or group residential setting is required in addition to a working knowledge of local service agencies. 30 hours per week. Bachelor level preferred but will consider associates with experience in linking services in Chemung County. Position is 5 days/30 hours/week with schedule flexibility possible. Benefits include paid time off. Rate of pay $20 - $25 an hour based on experience.

Duties:
- Develop and manage individual service plans for clients
- Coordinate and oversee behavioral health services
- Provide administrative support for program development initiatives
- Supervise and support staff in delivering high-quality services
- Manage budgets and ensure financial accountability
