Gilead Community Services is seeking a caring and recovery oriented APRN to join our multi-disciplinary behavioral health and substance use disorder treatment agency.

About the position:

The APRN for Gilead’s Old Saybrook area program works in our DCF Program with youth. The position works primarily in our Old Saybrook location for 15 hours/week (flexible between 1st and 2nd shift hours, Thursday afternoons mandatory) with the opportunity for additional hours. Clinical knowledge regarding severe and prolonged mental health and substance use disorders as well as medical knowledge regarding health and well-being are required.

Minimum Requirements

• Must be a licensed APRN in CT
• Experience working with individuals with substance use disorders and psychiatric issues
• Five years of clinical experience
• Must maintain and provide proof of valid driver’s license and automobile insurance in good standing.
• Reliable use of personal vehicle as needed

This is a non-union position and pay isdependent on level of education and experience.

Responsibilities:

• Providing prescriber services and medication management to adults with severe and prolonged mental health and substance abuse disorders, including those who require MAT
• Conducting psychiatric evaluations and Mental Status Exams for all newly admitted clients as well as annual visits
• Coordinating client care with the treatment team, including program RN’s, clinicians, and residential program staff
• Maintaining contact with MDs (i.e., PCPs, specialists, etc.) to coordinate medical and psychiatric care of individuals
• Working closely with pharmacies to facilitate proper pharmaceutical care
• Providing ongoing medical education to case management and program staff, both informally and in trainings
• Consulting with the Psychiatrist regarding medical and mental health needs as well as risk management concerns
• Making appropriate community referrals based on assessments and evaluations to meet clients’ basic health needs
• Completing all necessary documentation in a thorough and timely manner
