JOB SUMMARY

The Case Manager II reports to the Regional Director and is responsible to assist in the leadership and coordination of activities and to develop the day-to-day program operations of a Permanent Supportive Housing Program.

ESSENTIAL JOB RESPONSIBILITIES

  1. Perform intake activities, including verification of homelessness and disability according to HUD definitions.
  2. Provide residents with instruction, as related to specific supportive needs.
  3. Assure that the residents understand and follow program rules and procedures, by explaining rules and procedures to them and making sure each resident has a copy.
  4. Complete documentation as needed to include specialized incident reports, Crisis intervention, needs assessments, and individualized service plans.
  5. Maintain relevant relationships with all referral sources.
  6. Coordinate resident activities and socialization events.
  7. Linkage with community resources focusing on housing.
  8. Will transport clients to and from appointments.
  9. Work cooperatively with MAP (Multi-Agency Program).
  10. Observe and address correction of assigned household duties including all safety issues.
  11. Become familiar with the “California Tenant Laws” and how they relate to the HUD Permanent Housing Program. Become familiar with how to enroll a resident which includes the rental agreement, HUD’s 30% rent calculation, the program participation agreement and all other HUD related documentation including entry in the HMIS data system.
  12. Become familiar and practice Housing First principles to providing services.
  13. Physical presence at the program is required.

ESSENTIAL JOB REQUIREMENTS

  1. 4 year bachelor degree, OR 2 year associates degree, OR 2 years of experience in the human services related field.
  2. Experience in a Mental Health residential setting, with an emphasis on supportive housing services preferred.
  3. Ability to work effectively with program residents, who are diagnosed with chronic mental illness.
  4. Current certified training in emergency first aid procedures and CPR preferred.
  5. Intermediate computer skills in Microsoft Word, Excel, Outlook and PowerPoint, Office 365, Teams, Zoom and other communication/meeting platforms Possess a valid driver’s license and have access to a dependable means of transportation that is properly insured and operated in accordance with all laws. Agency insurer authorization is required.
  6. Ability to pass a criminal background check.
  7. Ability to pass a pre-employment drug screen and TB test.
