Summary/Objective: An employee working as Community Health Worker with the Shasta Cascade Health Centers is responsible for assisting patients and their families navigate and access community services, health services, and adopt healthy behaviors. The CHW will work collaboratively with patients and their families, community resources, county programs and clinical staff as appropriate. The CHW will also identify barriers to care and share that information with Leadership for quality improvement.
Essential Functions:
1. Ability to maintain confidentiality
2. Support public health efforts to educate the community on infection diseases
3. Deliver information about health and wellness in ways that the community can easily understand
4. Establish positive and supportive relationships with patients and those who work within the community/county programs to foster positive experiences and outcomes
5. Motivate patients to be active and engaged in their health
6. Ability to objectively report data and identify targeted areas of program activity
7. Ability to document activities as required within the provided systems
8. Other duties as assigned

Community Health Worker Responsibilities:
The CHW will instrumental in building the Enhanced Care Management program for Shasta Cascade Health Centers. This role will be front facing with patients and families in order to determine if patient(s) qualify for the ECM program and then support in accessing appropriate programs for their needs. This position requires a majority of patient facing time in the community, as well as time spent collaborating with the various programs in the community that may serve as resources for patients, such as community resources centers, food banks, mental health services, public health services and more.
Work Environment:
This job operates in multiple environments:
1. a professional medical clinic environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
2. This position will also operate in the community which will be a variety of indoor and outdoor settings; ie: community/senior centers, community organizations, resource fairs, etc.
Travel:
Travel is expected for regular job duties. Travel may occasionally be required for training purposes. If personal vehicle is used for specified job-related errands or travel to/from training events, mileage will be reimbursed as specified in the Employee Handbook.
Qualifications
Education, Experience, Certifications, Licenses and Registrations:
1. High School Diploma or GED required
2. Two years working in social work, community outreach, or customer service
3. Must successfully complete an approved Community Health Worker program within 12 months of hire
4. Basic Life Support (BLS) certification, or ability to become certified.

Physical Requirements:

While performing job duties, the employee is regularly required to talk and hear. The employee is frequently required to stand, walk, reach with hands and arms, and use hands and fingers.

· Ability to lift or move objects weighing up to 50 pounds.
