Overview:
Bachelor’s degree in social work or a bachelor’s degree in a human services field including but not limited to sociology, special education, rehabilitation, counseling, and psychology.
One year of supervised social work experience in a health care setting working directly with individuals, or comparable experience.
Provide medically related social services to assist the resident to attain or maintain their highest practicable physical, mental and psychosocial well being, and prevent the occurrence or progression of personal and social problems.
Arrange for social services through health and welfare resources in the community and for the integration of social services, with other elements of the resident’s plan of care.
Conduct admission and ongoing assessments of resident’s relative to psychosocial problems /needs and establish appropriate interventions. Initiate discharge plan day one of admission and periodically update the discharge plan on all residents; establish and implement referrals to appropriate community services when a resident is discharged.
Conduct ongoing evaluation of quality and appropriateness of social services provided.
Maintain all required documentation for social services.
Maintain good interpersonal relationships with nursing and facility personnel, residents, family, visitors and community agencies.
Act as role model, leader, and innovator.
Promote public relations and quality assurance.
Maintain compliance with Federal and State regulations for Social Services in Long Term Care Facilities.
