Centerpoint Health is Hiring a Program Coordinator Near Middletown, OH

The Program Coordinator position at Centerpoint Health In Middletown and Franklin Ohio, supports the mission and vision of Centerpoint Health by working directly with patients to help them navigate and access community services while adopting healthy behaviors with an aim to prevent HIV and increase early access to HIV treatment. They will promote, maintain, and improve individual and community health through screenings and data collection and monitoring. This position is grant funded and depending on future funding will determine the longevity of the position.
Program Coordinators provide creative strategies, materials, and tools for training, educating, and changing lifestyle behaviors and are active promoters of health in their community.
Job Duties:
  • Educate and link individuals, families, groups, and communities to resources including health insurance, food, housing, quality care, HIV care management, community resources, and healthcare information.
  • Provide culturally appropriate health education and information.
  • Hold or staff a table at workshops, group discussions, and community events to learn and advocate about ways the community can promote good health.
  • Assist patients and families in problem-solving to improve their ability to access integrated healthcare.
  • Conduct community outreach to promote access to care.
  • Conduct appropriate screenings.
  • Contact, schedule, and track follow-ups with patients.
  • Arrange transportation for healthcare appointments.
  • Advocate for patient and community health needs.
  • Provide informal counseling and guidance on health behaviors.
  • Conforms with and abides by all regulations, policies, work practices and instructions.
  • Instrumental in closing the referral loop for HIV care.
  • Communicate with providers as needed to obtain input on coordination of care issues.
  • Collaborate with grant management team on planning and implementation.
  • Complete accurate and timely documentation.
  • Assist with grant data collection, reporting, and meeting grant outcomes.
  • Able to travel between all clinic sites – Middletown, Franklin, and Norwood.
  • Other duties as assigned.
Certifications/Credentials:
High school diploma.
Associates degree preferred.
Community Health Worker experience preferred.
Other Skills and Qualifications:
  • Able to spend significant time standing and walking.
  • Able to use equipment required for the delivery of services.
  • Detail-oriented.
  • Possess strong working knowledge of data collection. Familiarity with statistics preferred.
  • Ability to work collaboratively within an integrated health care environment.
  • Demonstrate strong oral and written communication skills when interacting with patients, families, and other health care providers.
  • Ability to work with interruptions, manage priorities, and work unsupervised.
  • Ability to be adaptive, flexible, relatable, and responsive to the needs of the health center and the needs of patients.
  • Proficient computer skills, including Microsoft Office. Previous knowledge of Electronic Medical Records preferred.
