MDS Coordinator, RN Summary:

• The role of the MDS Coordinator, RN is to coordinate the development and completion of the resident assessment in accordance within the requirements of the state / government regulations and facility goals and policies.
• The role of the MDS Coordinator, RN is to coordinate the development & completion of the resident assessment in accordance with the requirements
• Coordinate the development & completion of the resident assessment in accordance with the requirements of the regulations, facility goals, & policies
• Participate in Continuous Quality Improvement
• Audits MDS, CAAS, PPS, QRP, & QM reports for continued compliance
• Participate in facility surveys - NYSDOH, OSHA, Infection control, Safety etc.
• Conduct / coordinate the interview of each resident for the resident assessment instrument & ensure they are completed
• Obtain medical records to assure correct information for accurate completion on MDS / SNF PPS
• Ensure that cut letters are provided to resident/representative as per regulations
• Communicate / coordinate significant change, determine skilled level of care & assure OMRA documentation as needed
• Assure assessment reference dates ARD are based on regulatory timeframe for completion of assessment & captures intensity of care required by the resident to ensure payment at the RUG level entitled
• Clearly communicate alert to change in payer status & compliance with transmission / submission targets
• Ensure compliance on Med A Certification Forms

1. Must be a graduate of an approved school of nursing
2. Current license in NYS as RN, Registered Nurse
3. Minimum of 3 years’ experience in hospital or long-term care
4. 1 year experience with MDS, PRI, & CCP
5. MDS certification & PRI certification, preferred
6. Willingness to work harmoniously with others & do the job with detailed focus & able to continually learn current knowledge as it relates to LTC & MDS
7. Excellent interpersonal & organizational skills
8. Team player with clinical skills & the ability to communicate effectively with diverse individuals
