QUALIFICATIONS
1. Licensed to provide occupational or physical therapy in the State of Florida.
2. Bachelor's Degree (Master's preferred) in Occupational Therapy or successful completion of the Certified
Occupational Therapy Assistant (COTA) Career Mobility Program through the American Occupational Therapy
Association.
3. Bachelor's Degree (Master's or Doctorate degree preferred) in Physical Therapy.
4. Satisfactory completion of an internship or experience in the area of occupational or physical therapy.
5. Valid Florida Driver License.
