Essential Duties

• Provide one-on-one skilled nursing care to patients in their homes (e.g., wound care, catheter care, medication administration, symptom management, etc.)
• A focus on admission visits
• Provide patient and family/caregiver support and education
• Practice cohesive and collaborative care with the healthcare providers
• Additional opportunities to earn for on-call availability

Requirements and Qualifications

• Registered Nurse (RN) licensure
• Prior Home Health and/or Hospice experience preferred, but not required
• Ability to work independently and demonstrate critical thinking
• Current BLS certification from AHA
• Be a licensed driver with an insured automobile in good working order
• Home Health or Hospice experience is strongly preferred. If no experience, should be willing to consider full-time availability for training.
