BASIC PURPOSE:The Case Manager is responsible for providing high quality care in the program in which they work. The Case Manager is responsible for utilizing good skills in assessment, treatment planning, case management, group planning and facilitation, and the documentation of services. The Case Manager is expected to function well as a member of the Clinical Team. Responsible for providing targeted case management services and assessments, service planning and evaluation, linking, advocacy, and crisis intervention to assigned case load. Responsible for providing individual, family, marital, and group therapies.
QUALIFICATIONS:1. Bachelor’s Degree in human service field. 2. Experience in behavioral health is strongly preferred. 3. Required documentation must be current and in compliance with Medicaid and licensure standards. 4. Must maintain a favorable rating on performance evaluations. 5. Must have a valid driver’s license and a clean driving record.
PHYSICAL REQUIREMENTS:-Walking, standing, stooping, kneeling, reaching, pushing, and pulling, lifting, carrying and grasping are necessary body movements utilized in performing duties.-Must be able to read and write legibly in English.-Hearing within normal range is required.-Visual acuity must be within normal range.-Must have manual dexterity for using equipment as necessary.-Must be able to operate a motor vehicle.MENTAL REQUIREMENTS:
-Must have ability to maintain client confidentiality.-Must have the ability to work successfully under potentially stressful conditions, and must be capable of adapting to varying workloads on a constant basis.-Must have the ability to make sound, independent judgments, and also be able to collaborate with members of the interdisciplinary teams, clinical staff and physicians, and other members of the agency in an appropriate fashion.-Must have the ability to comprehend and perform oral and written instructions and procedures.-Must have effective reading and comprehension skills.-Must have strong communication skills, written and verbal.
ENVIRONMENTAL CONDITIONS: (POTENTIAL FOR)-Exposure to chemicals, such as cleaning products and other products used in maintaining the facility.-Exposure to stress and constant interruptions.-Possible exposure to body fluids, contagious diseases, etc. while carrying out services.
