Cherrelyn Healthcare Center is a 189-bed skilled nursing facility located in Littleton, CO. We have been operating for over 50 years and are looking for an individual to continue the tradition of the best in care for our residents.

Compensation & Benefits:
This is a full time, hourly position that pays $30-$33 per hour, paid bi-weekly. Cherrelyn Healthcare Center also offers a competitive benefits package that includes PTO, health insurance, vision, dental, and 401K.
Responsibilities:
• Develop and lead social service programs and activities
• Assess needs, evaluates available resources, and formulates strategies to achieve goals
• Develop and coordinate social work services with other healthcare team members
• Provide case management services and support to patients, families, and caregivers
• Provide crisis intervention and suicide prevention services
• Conduct psychosocial assessments to identify risk factors
• Develop care plans and coordinate treatment plans
• Educate and train staff, families, and patients on various services
• Participate in treatment teams or other healthcare meetings
• Maintain patient records and documentation according to established guidelines
Requirements:
• BS or MS in Social Work required
• Licensure in social work with the state
• Three years of experience in direct social work services
• Excellent communication, interpersonal, and organizational skills
• Strong knowledge of case management and patient advocacy
• Knowledge of community resources and services
• Ability to work independently and as part of a team
