Fair Oaks Health Care Center is Hiring an ADMISSIONS NURSE (RN) Near Libertyville, IL

Fair Oaks Health Care in Crystal Lake is currently looking for an Admissions Nurse. Come join our team. Schedule for this position would be Monday through Friday (12:30 pm - 9pm) and would participate in the on-call rotating schedule.
Job Description
  • Complete admissions per admission process policy
  • Performs assessments up to 72-hours after admission. Including initiation of the discharge summary on admission
  • Documents according to current standards and established policies
  • Develops and implements appropriate plan of care based on assessment and diagnosis tailored to meet everyone's needs (physical, cultural and psychosocial), continually evaluating plan of care to make necessary changes, works to meet individual patient/resident needs
  • Attends and participates in resident/patient care conference
  • Observes for significant changes in condition for the purpose of MDS
  • Communicates issues daily to coordinate the interdisciplinary process to include documentation, care plan and resident's concerns
  • Completes all necessary and required forms/documentation in a timely manner
  • Performs in the role of clinical manager as necessary
  • Must be willing to assist other nursing staff in daily shift duties as needed
  • Must model effective communication between residents, family, staff and community
  • Provides patient/resident, family and caregiver education, identifies learning needs, clearly communicates information and documents patient/resident response to education
  • Plans for patient/resident discharge, anticipates and plans for long-term and/or short-term patient/resident needs, collaborates with the discharge planner when appropriate and assesses the patient/resident prior to discharge
  • May need to work as floor nurse as dictated by the needs of community
  • Demonstrates knowledge of medications and their correct administration based on age of patient/resident based on age, and clinical condition. Evaluates responses to medications
  • Responsible to know, implement and assure compliance to State, Federal and/or County regulations and CHC policies and procedures
Qualifications Required
  • Current valid state licensure as a Registered Nurse, in good standing
  • Must be English literate and possess excellent written and verbal communication skills.
