Role Description

This is a full-time on-site role for a Licensed Practical Nurse. The Licensed Practical Nurse will be responsible for providing personalized care and support to residents with Alzheimer's and dementia. Day-to-day tasks include administering medication, monitoring vital signs, assisting with activities of daily living, and collaborating with the care team to develop personalized care plans.

Qualifications

• Valid Licensed Practical Nurse (LPN) license
• Experience in providing care for individuals with Alzheimer's and dementia
• Knowledge of medication administration and dosage calculations
• Ability to effectively communicate and collaborate with residents, families, and the care team
• Strong observational and critical thinking skills
• Compassionate and patient-centered approach to care
• Ability to work well in a team environment
• CPR certification PREFERRED
