Description

Overview:  Provide nutritional support for residents. Assess the nutritional needs of residents and develop individualized care plans. Monitoring dietary restrictions, providing nutrition education, and collaborating with other health care professionals to ensure residents’ overall well-being.

Qualifications: Bachelor’s or Master’s degree in nutrition or Dietetics. Registered Dietician (RD) credential, State licensure (if required). Experience in long-term care or a similar setting is preferred. Strong communications and interpersonal skills. Experience with electronic charting and nutrition care process. Ability to work effectively in a team environment.

Duties and Responsibilities: Duties are determined by the company’s needs and may include, but are not limited to, the following:

  • Coordinate menu planning, purchasing, and food preparation by assisting in appropriate procedures, specifications, and recipes.
  • Through observation and evaluation, promote food production and service procedures that conserve nutritive value, flavor, appearance, and quality, and are attractively served at the proper temperature and in a form to meet individual needs. Complete MDS section.
  • Review and assess all initial and annual nutritional assessment reviews and care plans.
  • Review and assess all nutritional high-risk charting.
  • Keeping accurate records of residents’ nutritional assessments and progress.
  • Monthly QAPI reports, significant weight changes, kitchen sanitation inspections, staff development, and dining programs.
  • Monitor the dietary restrictions and requirements of residents.
  • Provide clinical support for CDM/Culinary Director.
  • Be knowledgeable of federal, state, and community’s rules, regulations, policies, and procedures.
  • Conduct monthly kitchen sanitation inspections.
  • Develop and conduct in-service training for dietary personnel and as needed, for other healthcare community personnel.
  • Understand community’s fire and disaster plans, follow established procedures during drills and actual emergencies.

  • Manage time efficiently.

Attendance and Reliability:

  • Rounds at communities per contracted schedule.
  • Provide appropriate notice in advance to communities of absence.

Other:

  • Maintain a well-groomed appearance that follows state Food Code standards.
  • Must be able to cope with mental and emotional stress.
  • Must be able to communicate and work effectively with various levels of staff, residents, family members and the public.
