Care Coordinator - Family Care of the Bluegrass - Lawrenceburg

Job Duties:

• Collaborate with provider teams to ensure all laboratory and radiology reports are received, reported to provider, and follow-up communication with patient, primary care provider, and/or specialty practice is documented.
• Collaborate with local facilities to ensure History & Physical reports, discharge summaries, and office visit notes are received prior to follow-up visit.
• Ensure information requested by patient is reported to primary care provider post-visit.
• Chronic Care Management (CCM)
• Run report of Medicare patients eligible for wellness exam quarterly
• 2 or more chronic diseases
• Contact patient to come in for visit with provider to set up CCM services
• Facilitate appropriate paperwork for consent for CCM services
• Utilize templates to document CCM services
• Referrals to specialty providers, home health, transitional care, etc.
• Contact with patient via phone/patient portal to discuss care plan, labs, etc.
• Med reconciliation
• Reviewing current care plan and making changes
• Reviewing medical records and test results
• Contacting other providers regarding patient care
• Chronic Disease Education
• Provide disease education and management
• Diabetes
• Hypertension
• COPD
• Other duties as assigned.

Qualifications:

• Preferred Associates degree in Nursing or higher
• If LPN/RN, valid license
• CPR certification
• Experience in medical office setting preferred
• Maintenance of required continuing education courses
