Are you passionate about making a tangible difference in the health and well-being of your community? We are seeking a dynamic and dedicated Community Health Worker (CHW) to join our team! As a CHW, you will be at the forefront of planning, implementing, and evaluating innovative educational programs that promote individual, family, and community health. Your efforts will focus on vital areas such as community health promotion, reducing infant mortality, and lowering unintended pregnancy rates, in alignment with the Division of Public Health Agreement Addendum 175 and Supporting Women’s Health Services. You will also manage program budgets, collaborate with mobile clinics, and work closely with individuals of reproductive age and their families to enhance maternal and infant health outcomes. If you are ready to take on a rewarding role that combines public health advocacy with hands-on community engagement, we want to hear from you!***This is a grant-funded time limited position eligible for County and fringe benefits. Salary is based on education and direct work related experience***
EXAMPLE OF DUTIES
Reproductive Health Coordinate with staff to implement evidence-based services in the selected Satellite clinic locations as outlined in Agreement Addendum 175 provided by the NC Women, Infant and Community Wellness Branch of the NCDPH.Facilitate and coordinate with staff to provide educational classes and materials that incorporate information on reproductive health, LARCs, Safe Sleep Practices, and maternal and infant health.Perform follow-up's on missed family planning appointments.Develop and administer a participant satisfaction survey to evaluate services and submit an annual satisfaction summary report to DPH.Collects data on the number of patients accessing contraceptive services during the satellite clinic, and report on an annual basis to DPH.Collaborates with other key agencies to provide education and facilitate events that promote infant mortality reduction, and reduction in unintended pregnancy rates.Develops advertisement of services, educational classes and events related to infant mortality reduction, and promotion reproductive health services.Submits interim and final program reports per the requirements listed in Agreement Addendum 175. Plans, develops and submits budget requests for the Supporting Women's Health program, and monitors budgetary activities.Maternal and Infant HealthEducate pregnant and postpartum participants about the 12-month postpartum Medicaid coverage, application process for Medicaid , and recertification for Medicaid.Provide education, specific to newborn care, development, feeding, safety. and the need for well baby care and immunizations. Make referrals based on participant need for resources.Collaborates with other key agencies to provide education and facilitate events that promote infant mortality reduction, and reduction in unintended pregnancy rates.Develops advertisement of prenatal and postpartum services, child health services, educational classes and events related to infant mortality reduction, and promotion reproductive health services.Collect data on program participants and service deliverables and report on an annual basis to DPH.Participate in all program webinars and meetings as required by DPH.OtherCollaborates with various churches, companies, agencies, organizations, health specialists, civic groups, and individuals to achieve health department goals, and any other duties as instructed by the Health Director
