JOB DESCRIPTION

Consultant Social Worker

REPORTING RELATIONSHIP:

Supervised by:
Positions Supervised:
Interrelationships: Physicians, Pharmacist, Dietary, Rehabilitation, and other health care team members

JOB SUMMARY:

The primary purpose of your job position is to assist in planning, organizing, developing, and directing our facility’s social service programs in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Administrator, to assure that the medically related emotional and social needs of each resident is met/maintained on an individual basis.

QUALIFICATIONS:

1.! Must possess, as a minimum, a Masters Degree from an approved school of Social Work.
Accredited by the Council on Social Work Education.

2.! Must have one (1) year experience in health care setting.

3.! Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.

4.! Must possess the ability to make independent decisions when circumstances warrant such action.

5.! Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.

6.! Must possess a working knowledge of regulations that govern nursing care facilities.

7.! Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level in which they are currently functioning.

8.! Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing social services.

JOB FACTORS:

Physical Requirements:
Job requires considerable physical effort most of the day including kneeling, squatting, and reaching, twisting, climbing, walking, exposure to temperature and humidity changes. Must able to push, pull, move, and/or lift a minimum of 25 lbs and able to, pull, move, and carry such weight a minimum of 50 feet.

Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position.

Mental Requirements:
Must be able to work independently, make judgments based on assessments and data available and act accordingly. Must be flexible, innovative and possess good interpersonal skills. Must be able to cope with mental and emotional stress and demonstrate emotional stability.

Working Conditions:
Be able to tolerate exposure to elements including, but not limited to, odors, blood, body fluids and excrements, adverse environmental conditions and hazardous materials.

ESSENTIAL FUNCTIONS:

345.! Assist in planning, developing, organizing, implementing, evaluating, and directing the social service programs of this facility.

346.! Participate in discharge planning, the development and implementation of social service care plans and resident assessments as necessary.

347.! Involve the resident/family in planning social service programs when possible.

348.! Assist in arranging transportation to other facilities when necessary.

349.! Refer resident/families to appropriate social service agencies when the facility does not provide the services or needs of the resident.

350.! Provide information to resident/families as to Medicare/Medicaid eligibility, and other financial assistance programs available to the resident.

351.! Provide consultation to members of our staff, community agencies, etc., in efforts to solve the needs and problems of the resident through the development of social service programs.

352.! Coordinate social service activities with other departments as necessary.

353.! Ensure that charted social service progress notes are informative and descriptive of the services provided and of the resident’s response to the service.

_ *354.! Participate in regularly scheduled reviews of resident discharge plans.
355.! Make rounds to assure that social services personnel are performing required duties, and to assure that appropriate social service procedures are being rendered to meet the needs of the facility.
356.! Monitor social services personnel to assure that they are following established safety regulations in the use of equipment and supplies.
357.! Assist in developing preliminary and comprehensive assessments of social service needs of each resident.
358.! Assist in developing a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident and the goals to be accomplished for each problem/need identified.
359.! Encourage the resident/family to participate in the development and review of the resident’s plan of care.
360.! Assist in the scheduling of care plans and assessments to be presented and discussed at each committee meeting.
361.! Participate in resident/group council meetings as requested and provide support services to such council.
362.! Make monthly inspections of all social service functions to assure that quality control measures are continually maintained.
363.! Provide written, dated, and signed reports to the administrator of each consultation visit.

364.! Provide a social care plan for each resident to assist in meeting the social needs of each resident.
