Actalent is in search of Licensed Clinical Social Worker in Kula, Maui. Pay is $50-60/hr.

Job Summary: Provides social work services including assessment, care planning, and case coordination to patients and their families. Provides consultation to staff and other health care providers. Initiates social work plan of care. Serves as liaison for community to integrate services, coordinate care for clients, and enhance relationships through public relations activities.

License, Certification, Education

• Masters degree in social work.
• Licensed Social Worker (Hawaii) OR Licensed Clinical Social Worker (Hawaii)
• Basic Life Support OR CPR/AED for Professional Rescuers

Additional Requirements:

• Healthcare experience working with medical diagnoses.
• Demonstrated ability in networking with community agencies.

Preferred Qualifications:

• Minimum one (1) year of social work experience.
