This position is for a Care Transition Coordinator Personal Care & Support Services in the Johnstown, PA area.

As a Care Transition Coordinator , you will be called to care when you’re needed most. As part of Interim HealthCare, you’ll provide support and guidance to patients who require continuum of care - ensuring continuity of care & service as patient lifecycle progresses.

As a Care Transition Coordinator , you will : ?? ?

• Provide daily onsite access to transitional care coordination
• Ensure timely assessment of patient and family needs prior to discharge
• Provide Information for Families & Patients to Manage Care
• Offer assessment and guidance to discharge staff regarding appropriate patient referrals for home care or other post-acute care
• Expedite referrals for any type of post-acute care
• Improve the Transition to Home Care Team
• Provide insights on post-acute care trends

To qualify as a Care Transition Coordinator with us, you will need a : ? ?

• Current unrestricted license to practice as a Licensed Social Worker (LSW) in the state associated with this position required at minimum
• One (1) year of professional experience practicing as a LSW, as listed above, required
• Reliable transportation to / from care sites and / or work locations
• Previous experience as a home care intake specialist or case manager preferred
