Position Title: Primary Care Physician
Department: PACE (Program of All-Inclusive Care for the Elderly)
Supervisor: PACE Medical Director
Status / Hours per week: Full-Time / 40 hours per week
Primary Function:
Responsible for the primary care of PACE participants.
Duties & Responsibilities:
• Acts as the primary care physician for PACE participants in community and institutional settings.
• Provides routine care, episodic, and urgent care.
• Performs initial and semiannual evaluations.
• Establishes collaborative practice with PACE nurse practitioner.
• In conjunction with nurse practitioner, performs routine. assessments and examinations of PACE participants.
• Together with the interdisciplinary team, develops care plans for PACE participants.
• Works collaboratively with members of interdisciplinary team.
• Communicates with consulting physicians and integrates recommendations into plans of care.
• Prescribes in accordance with state and federal regulations.
• Participates in on-call rotation.
• Attends interdisciplinary team meetings.
• Maintains appropriate medical records and related documentation.
• Maintains appropriate licensure and board certification.
• Practices in accordance with appropriate PACE guidelines.
Minimum Basic Knowledge:
• Experience working in an interdisciplinary team
• Comfort working in a multicultural health care environment
• Knowledge and comfort working in a managed care setting
Experience & Qualifications:
• Physician holding MD or DO degree and licensed to practice medicine in Massachusetts
• Interest and at least one-year of experience working with the geriatric population
Define Access Level to PHI: Level 4: Authorized to access full health information. UCHC position and job responsibilities involve the provision of patient care and working as a clinical team member. Staff in this category level, although allowed full access, should only access the necessary information for each respective treatment encounter/circumstance.
