The Social Services Director is an integral part the care team that helps change the lives of our patients and residents each day. As a Social Services Director you are responsible for providing care in an interdisciplinary environment including:

• Assesses and identifies medical related social and emotional needs of the residents. Develops and implements social care plans with the involvement of the resident, their family, physician and other nursing care staff
• Consult with physicians and the healthcare team members in dealing with psychosocial factors relating to residents' illnesses, treatments, and discharge plans.
• Provide discharge planning, including referrals to community and family social services
• Assists residents with financial and legal matters, provides or arranges for counseling.
• Directly supervisor the Social Services Assistant
• Possesses knowledge of federal and state regulations, including Medicare/Medicaid to arrange for needed services not covered by state regulations.
• Develops and maintains community liaison activities.

Required Qualifications

• Bachelors degree in social Work
• Minimum of two years related supervisory experience and/or training.
• ACSW registration preferred
