Clayton County Board of Health is Hiring a Community Health Worker Near Jonesboro, GA

Job Summary:

The Patient Navigator /Community Health Worker will serve as a key resource for developing and expanding patient access to care resources throughout the Clayton County Health District CHLI. Will assist with community on identifying various health opportunities; work closely with physician offices and health clinics to enhance patient involvement in medical planning and resources. Communicate with clinical staff on arears of needed improvement; provide daily information to patients and providers, and monthly reports to PI/PD. PA/CHW serves as an advocate providing social support; informal counseling and individualized assistance to patients to facilitate access to quality care.

Primary Duties & Responsibilities:

  • Educate patients on health care opportunities and processes by establishing trusting relationships while strictly adhering to the policies and procedures.
  • Improve access to health care services; increase health and screening; understand between community members and the health and social service system.
  • Enhanced communication between community members and health providers; increase the use of health care services; improve adherence to health recommendations; and reduced need for emergency and specialty services.
  • Effectively work with people (staff, clients, doctors, agencies, etc.) from diverse backgrounds in reducing cultural and socio-economic barriers between clients and institutions
  • Identify potential clients, complete initial needs assessment, and maintain follow-up to ensure appropriate guidance to health services.
  • Assist clients with utilizing available resources, including scheduling medical appointments, accompanying clients to appointments, and assisting with completion of medical program applications.
  • Coordinate access to health-related services by assisting clients with utilizing available resources.
  • Facilitate and schedule patient appointments/referrals with appropriate team/ provider(s) for care needed.
  • Make confirmation calls/emails to patients regarding future appointment dates.
  • Respond to patient inquiries and assist patients with the completion of paperwork.
  • Develop relationships with community partners/ health care providers within and outside the community health center to allow for informed decision to be made regarding participants healthcare needs.
  • Develop and maintains strong working relationships with external (referral sources and community agencies) and internal contacts (departmental team members)
  • Assist team in the circulation of programmatic newsletter to clients and public.
  • Maintain accurate client records by documenting all contacts, services provided, and outcomes to track all clients and services for future reference.
  • Create files and enter data into computer system and generate required reports.
  • Facilitate, attend trainings and learning opportunities that expand content knowledge and best practices.

Minimum Qualifications:

Master’s Degree in Public Health education from an accredited college or university OR Bachelor’s degree in education, social, behavioral, or biological sciences or a directly related field from an accredited college or university.

Job Type: Part-time

Pay: $20.00 per hour

Expected hours: 1 – 29 per week
