TNC Community is Hiring a Community RN Near Independence, MO

Responsible for monitoring and ensuring that appropriate health care, specific to each individual, is identified and communicated to the TNC Community support team.
The Community RN will perform duties specified by the State of Missouri including monthly assessment of each assigned individual and completion of required reports. The Community RN advocates for the removal of architectural, environmental, attitudinal, financial, employment, communication, transportation, community integration, and other barriers for persons receiving services through TNC Community. Responsible for reporting all suspected/alleged abuse and neglect in accordance with TNC Community policies.

Education And Experience Requirements
  • Graduate from an accredited school of nursing; B.S.N. Degree and/or CDDN.
  • Certification is desirable.
  • Current registration with the Missouri Board of Nursing verifiable by primary source.
  • Thorough knowledge of general nursing and management practices.
  • Experience should include at least one year in nursing practice.
  • Experience of at least one year working with individuals with ID/DD preferred.
Responsibilities/Essential Functions
  • Be familiar with CARF Standards and DMH regulations, and monitor for implementation of services related to health and safety processes within those guidelines.
  • Prepare Annual Health Review for each individual.
  • Update Health Maintenance Schedule (HMS) annually.
  • Provide written or verbal input to DHS in advance towards plan development.
  • Complete Community RN requirements as designated in Community RN training, including face-to-face contact with each assigned individual monthly,
  • Review and monitoring of delegated procedures and completion of Community RN Summary report.
  • Includes but not limited to AIMS screening every 6months when indicated and choking risk assessment annual or as indicated, G-tube monitoring, diabetic management/blood glucose reporting, vital sign and weight monitoring minimum of monthly.
  • Review all medical appointment data and records in Therap. Update new information in health record such as new diagnosis, identified infections, etc. as needed.
  • Prepare, review and update as needed RN Delegation procedure documents for nursing care, deemed by the Director of Health Services to be outside the scope of general orientation training of direct care staff. Assist with training staff to competency. Assist in development and follow up of Nursing Delegation.
  • Provide supervisor with current calendar of scheduled work time each month.
  • Attend in-service and training programs to further enhance professional knowledge.
  • Provide back-up support with telephonic on-call for agency staff and Health Team as assigned.
  • Follow all TNC Community policies and procedures.
  • Other duties as assigned.
Additional Requirements
  • Ability to clear background checks and drug screenings to include marijuana.
  • Maintain current CPR Certification.
  • Maintain current Abuse and Neglect Training.
  • Maintain Gentle Teaching training or equivalent.
  • Must participate in all mandatory trainings and re-certifications as required.
  • Must have own transportation and provide proof of insurance.
  • Flexible schedule.
  • Be familiar with and able to utilize a computer efficiently.
TNC Community’s drug and alcohol testing programs comply with the Federal Transit Administration regulation 40 CFR Part 655.
