Location: Joint Base San Antonio (JBSA), Lackland & Randolph AFB
Position: Registered Nurse, Case Manager - Airman Medical Transition Unit (WHASC)
Qualifications:
Education: Bachelor of Science in Nursing from an NLNAC or CCNE accredited institution.
Experience: Minimum 2 years in utilization management or broad-based clinical nursing and case management within the last 3 years. Required 30 hours of continuing education in nursing within the last 3 years.
Licensure: Current, unrestricted RN license.
Responsibilities:
Participate in and ensure compliance with the Case Management Program (CMP).
Develop and implement case management tools and strategies.
Coordinate care across diverse healthcare settings to optimize patient outcomes.
Document care accurately and coordinate patient education and follow-up.
Maintain liaison with community agencies and implement treatment plans.
Schedule:
Variable hours based on unit needs.
