Overview:
We are seeking a Certified Community Health Worker to join our team. As a Community Health Worker, you will play a vital role in promoting health education and providing support to individuals within the community.
Responsibilities:
Skills:
● High school diploma or equivalent
● Valid driver’s license and insured vehicle
● 90-hour Community Health Worker Certification, or ability to become certified
● Minimum of two years experience in a health or social service-related field preferred
● Strong interpersonal and communication skills
● Empathetic and culturally sensitive approach to working with diverse populations
● Knowledge of local healthcare resources and social services
● Proficiency in data collection and record-keeping
● Ability to work independently and as part of a team
● Familiarity with medical terminology and healthcare system, including Medicaid
(Oregon Health Plan) and Medicare
● Bilingual and/or bicultural preferred
