Columbia Gorge Family Medicine is Hiring a Community Health Worker Near Hood River, OR

Overview:

We are seeking a Certified Community Health Worker to join our team. As a Community Health Worker, you will play a vital role in promoting health education and providing support to individuals within the community.

Responsibilities:

  • Completes the Community Health Worker (CHW) protocols through a combination of office-based and home-visits for care coordination.
  • Assesses the health and social needs of patients using the CHW tools.
  • Elevates concerns to the patient’s Primary Care Provider or appropriate clinician.
  • Collaborates with healthcare providers to develop care plans and make appropriate referrals for medical, social, and support services.
  • Provides culturally competent health education and information to patients, families, and caregivers.
  • Ensures patients are aware of available community resources, make appropriate referrals, and assist patients in completing applications and accessing resources as appropriate.
  • Advocates for patients within the healthcare system to ensure they receive appropriate care and services in line with what matters most to them.
  • Supports patients in understanding their rights and responsibilities as healthcare consumers.
  • Offers emotional support and crisis intervention for patients facing health-related challenges.
  • Connects patients to mental health services or crisis hotlines as needed.
  • Maintains accurate records of patient’s interactions, services provided, and outcomes in the clinic’s Electronic Health Record (EHR) system.
  • Completes required evaluation forms for all referred patients.
  • Participates in evaluation surveys and meetings to continuously improve the program design.
  • Participates in regional networks to share with and learn from other CHWs.
  • Collaborates with local organizations and community leaders to identify and advocate for improved patient services.
  • Maintains punctual, regular and predictable attendance.
  • Works collaboratively in a team environment with a spirit of cooperation.

Skills:

● High school diploma or equivalent

● Valid driver’s license and insured vehicle

● 90-hour Community Health Worker Certification, or ability to become certified

● Minimum of two years experience in a health or social service-related field preferred

● Strong interpersonal and communication skills

● Empathetic and culturally sensitive approach to working with diverse populations

● Knowledge of local healthcare resources and social services

● Proficiency in data collection and record-keeping

● Ability to work independently and as part of a team

● Familiarity with medical terminology and healthcare system, including Medicaid

(Oregon Health Plan) and Medicare

● Bilingual and/or bicultural preferred
