Description :
Fee for service position with potential to become full time employee with benefits
DUTIES AND RESPONSIBILITIES :
1. Provide counseling services to members in Drug & Alcohol program
2. Complete Intake, treatment plans, evaluations and discharges on time as prescribed in the policies
3. Provide Individual and group therapies consistently and family therapy when needs
4. Use ASAM evaluation at the beginning and at every treatment plan sessions
5. Complete documentation on time as prescribed in the policies and procedure manual
6. Ensure all members provide UDS every week without fail
7. Apply for BHSI for clients who doesn't have insurance
8. Co-ordinate with Septa logistics care for bus passes for members to come to facility
9. Co-ordinate with Psychiatrist for evaluations, suboxone prescription for members and case consultations
10. Work closely with supervisor and complete required supervision hours and case consultations
11. Ensure all documentation and data are complete and in compliance with APM, city, state, and federal regulations
12. Makes referrals to appropriate levels of care based on ASAM criteria
13. Maintains strong interagency community, multidisciplinary and government agencies relationships.
14. Maintains ongoing communication with other community treatment facilities, primary care physicians, hospitals, schools, multidisciplinary units, and Probation / Parole officers, the Forensic Intensive Recovery (FIR) unit, etc. as necessary.
15. Maintains a caseload that supervisor deemed appropriate in your full time position
16. Participate in meetings with the agency interdisciplinary team as clinically necessary.
17. Participate and maintain required CE credits and other mandatory trainings
18. Ensure all charts are up to date
19. Actively engage in program expansion and development
20. Works closely with the Clinical Director and set quarterly targets and achieve them
21. Must be able to provide training and supervision if more counselors are hired for DA program
23. Provides updated FBI, PA criminal background, child abuse, CPSL and CPR clearances before the start date
25. Performs other program related duties as assigned by the Clinical Director.
