Wound Care Nurse Job Description

Responsibilities for wound care nurse

Collaborates with other health care providers, Wound Care Center Physicians, Program
Responsible for wound care
Maintains ongoing follow-up evaluation for appropriate ostomy and/or skin care patients with needs after discharge from the hospital to provide for continuity of care
Provides a safe environment for patients, visitors and staff and complies
Round Weekly on all wounds in hospital
Patient and Staff education
Assist with completing Admissions on Daily basis
Makes home visits to provide clinical support and assists nursing staff in patient assessment, identification of diagnosis, and development of a comprehensive nursing care plan
Manages a client caseload as needed
Provides consultation to staff identifying and planning interventions designed to solve complex problems involving client care
