Nurse Family Partnership is Hiring a Public Health Nurse Near Hennepin, MN

Description

Hennepin County is seeking a full-time Public Health Nurse to work with pregnant and parenting clients and their children enrolled in the Nurse-Family Partnership (NFP) Program. This evidence-based program provides eligible families home visiting services through pregnancy and until their child is 2 years old. We are looking for applicants who are compassionate, have strong communication skills, and are committed to health equity. A successful candidate will be organized, flexible, and self-directed with strong documentation skills. Clinical skills used will include pregnancy, postpartum, newborn, pediatric and mental health assessments, and education. This position is part of a team of nurses that is supported with weekly meetings with supervisor, monthly group reflective practice, and biweekly team meetings. Location and hours:
This position is hybrid and will be performed at our North Minneapolis Human Services Center at 1001 Plymouth Avenue, Minneapolis, MN 55411, in the field and out in the community, and remote as job duties require. Work hours will be Monday through Friday, 8 a.m. to 4:30 p.m. with some flexibility. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.

New employees who are hired into remote or hybrid positions between January 2, 2022, to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.

In this position you will:

  • Deliver family home visiting services, in accordance with model and funding requirements, and stay current with changes to rules, requirements and model standards.
  • Develop therapeutic relationships, utilize concepts of reflection and motivational interviewing with caregivers and their families in a home visiting environment.
  • Analyze assessed data and assist individuals to identify relevant goals along with specific supports that will enhance their ability to meet goals.
  • Develop and maintain community relationships to support client referrals and link families with community resources that are relevant to their specific needs.
  • Maintain appropriate, thorough, timely, and compliant documentation on visits into an electronic database.
  • Ensure services are resident-centered, culturally appropriate, and equity focused.

Need to have:

  • Licensure as a Registered Nurse issued by the state of Minnesota.
  • Certification as a Public Health Nurse by the Minnesota Board of Nursing.
  • A bachelor’s degree or higher in nursing from an accredited school of nursing and six months or more of full-time equivalent nursing experience.
  • A valid driver’s license and daily access to reliable transportation to travel within and outside the metro area to meet with people in their residential settings and/or perform authorized off-site work duties.

Nice to have:

  • One or more years of experience in family home visiting services, obstetrical nursing, pediatric nursing, or community health.
  • Experience working within a team, organization, or community to reduce health disparities.

Candidates with fluency in another language(s) in addition to English are encouraged to apply. Bilingual candidates who pass a language assessment may qualify for additional bilingual pay and duties.
