Job Overview:
We are seeking a compassionate and dedicated Medical Social Worker to join our team. This position is per diem or part time. As a Medical Social Worker, you will play a crucial role in providing support and assistance to patients and their families during their medical journey in Hospice or Palliative Care. Your primary responsibility will be to assess patients' social and emotional needs and develop appropriate care plans and assist with accessing community resources.

Duties:
- Conduct comprehensive assessments of patients' social, emotional, and environmental factors
- Collaborate with healthcare professionals to develop individualized care plans
- Provide counseling and emotional support to patients and their families
- Assist patients in accessing community resources and support services
- Advocate for patients' rights and ensure their needs are met
- Maintain accurate and up-to-date medical documentation
- Collect data and participate in research activities as required
- Provide education to patients, families, and caregivers on available resources and coping strategies
- Collaborate with interdisciplinary teams to coordinate patient care

Experience:
- Bachelor's or Master's degree in Social Work or related field
- Previous experience in a Hospice care environment
- Familiarity with assisted living facilities and special education programs is a plus
- Strong clinical counseling skills, including crisis intervention and grief counseling
- Excellent communication and interpersonal skills

Job Types: Full-time, Part-time, Per diem, PRN

Pay: From $60.00 per hour

Expected hours: 15 – 30 per week
