Looking for a dynamic, detail-oriented, and experienced Contract-to-Hire Case Manager to join our team. The ideal candidate should have a strong background in client intake processes, managing programs, and handling confidential client files. This is a contract position that will require you to work closely with our clients, providing them with exceptional service and support. You will be responsible for managing a caseload of clients, coordinating various services, and ensuring that all client information is kept confidential and secure.
Responsibilities
- Conduct comprehensive client assessments to identify needs, goals, and potential barriers.
- Develop, implement, and review individualized case management plans.
- Coordinate and provide access to a range of services including healthcare, social services, and legal aid.
- Maintain accurate and up-to-date client files, ensuring all information is kept confidential.
- Monitor and track client progress towards their goals, adjusting case plans as necessary.
- Advocate for clients, liaising with other professionals and service providers as needed.
- Participate in team meetings, providing updates and feedback on client cases.
- Ensure all case management activities comply with professional standards and organizational policies.
- Provide crisis intervention as needed, and refer clients to appropriate resources.
- Provide regular reports on case management activities, outcomes, and client satisfaction.
- Bachelor's degree in social work, psychology, or related field. A Master's degree is preferred.
- A minimum of 2 years' experience in case management, preferably in a non-profit or social services setting.
- In-depth knowledge of the client intake process, program management, and maintaining confidential client files.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with clients and stakeholders.
- Strong organizational and multitasking skills, with the ability to manage a large caseload effectively.
- Proficiency in using case management software and other computer applications.
- Ability to work independently, make decisions, and solve problems effectively.
- Knowledge of community resources and services for vulnerable populations.
- Ability to handle sensitive information with the highest degree of integrity and confidentiality.
- Certification in case management is a plus.
