Job Title: Home Health Registered Nurse (RN)
Job Type: Full-Time
Key Responsibilities
  • Primary Caseload Management: Oversee a primary caseload of home health patients, providing skilled nursing care as prescribed by physicians and in compliance with Medicare Conditions of Participation, PA licensure laws, and agency policies.
  • Comprehensive Nursing Assessment: Conduct thorough assessments of patient needs and collaborate to develop personalized care plans. Implement, evaluate, and revise care plans to ensure comprehensive and responsive interventions.
  • Team Collaboration: Work closely with other team members to meet all patient care needs, ensuring seamless and holistic care.
Primary DutiesAssessing and Planning:
  • Conduct comprehensive patient assessments with accuracy, compassion, and timeliness.
  • Utilize case management techniques to develop individualized care plans, including problem identification, interventions, and goals.
  • Identify both active and potential patient needs affecting the care plan.
  • Collaborate consistently with physicians to plan and implement medical and nursing care.
  • Participate in case conferences and contribute to patient care planning.
Implementing And Evaluating
  • Provide nursing interventions and appropriate preventative and therapeutic procedures.
  • Collaborate with team members for continuous patient assessment, interpreting findings, initiating therapeutic actions, and evaluating results.
  • Perform specialized procedures when qualified with the necessary training, competency, clinical background, and expertise.
  • Work with family/caregivers and team members to address the physical and emotional needs of patients.
  • Provide information and explanations to patients and families, facilitating education to promote continuity of care and optimal outcomes.
  • Demonstrate expertise in utilizing agency services and community resources.
  • Meet productivity requirements based on patient acuity and Clinical Manager directives.
  • Maintain accurate, complete, and timely clinical documentation.
Qualifications
  • Graduate of an accredited registered nursing program.
  • Bachelor’s degree in Nursing preferred.
  • Minimum of one-year general nursing experience; current home health, medical, surgical, or critical care experience preferred.
  • Ability to work effectively in a Home Health setting.
  • Active RN license in the Commonwealth of Pennsylvania.
  • Valid driver’s license and reliable transportation insured per PA state requirements.
  • CPR certification.
  • Satisfactory references from employers and/or professional peers.
  • Clear criminal background check.
