Hospital Liaison Responsibilities:

• Develop and maintain relationships with hospital discharge planners, case managers, and other healthcare professionals to facilitate patient referrals.
• Coordinate the admission process for patients transitioning from hospitals to our nursing home, ensuring a smooth transition of care.
• Educate hospital staff and patients' families about the services and amenities offered at our facility.

Hospital Liaison Qualifications:
