Description

BASIC PURPOSE:

Provide direct resident care under direct supervision of Licensed Practical Nurse (LPN) and/or Registered Nurse (RN). Identifies and meets resident’s needs staying in compliance with all hospital policy and procedures.

ESSENTIAL FUNCTIONS FOR CNA/RESTORATIVE AID

PHYSICAL REQUIREMENTS:

  1. Frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, grasping, and feeling are necessary body movements utilized in performing nursing duties throughout the work shifts.
  2. Must be able to lift, move, and position patients of all weights, with varying amounts of assistance in a fair manner on a regular and recurring basis.
  3. Must be able to read and write legibly in English.
  4. Hearing within normal range is required.
  5. Visual acuity must be within normal range.
  6. Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Light Work.

MENTAL REQUIREMENTS:

  1. Must have the ability to work successfully under highly stressful conditions, and must be capable of adapting to varying workloads and work assignments on a constant basis.
  2. Must have the ability to comprehend and perform oral and written instructions and procedures.
  3. Must have effective reading and comprehension skills.
  4. Must be able to work flexible hours, weekends, and holidays.

BASIC FUNCTIONS:

  1. Perform direct resident care, applying good judgment and knowledge of skills required within the realm of job responsibilities 100% of the time.
  2. Report all pertinent observations and concerns to LPN or RN.
  3. Document appropriate and correct information on resident’s flow sheets.
  4. Demonstrate competency in clinical and technical skills, complete skills checklist and correctly perform skills daily.
  5. Demonstrate ability to position and transfer residents in a safe manner.
  6. Demonstrates knowledge of restraint policy.
  7. Assisting resident to activities and assisting with activities if needed.
  8. An active participate in the Restorative Nursing Program including the Bowel and Bladder Programs.
  9. Efficiently use time to accomplish assignments, as well as ability to accept and respond to change in work environment in a way that promotes the facilities goals and objectives.
  10. Willing to comply with handbook on call off, confidentiality, and vacation policy.
  11. Will prioritize task and complete work in a timely manner
  12. Will treat all residents, family, visitors and coworkers with dignity and respect.

JOB DUTIES

ADMISSIONS

  1. Prepares room for new admission, orients resident and family to room, obtains vital signs, height and weight, and assures optimal resident comfort.

INTAKE AND OUTPUT

  1. Assist in maintaining fluid balance by accurately measuring and recording intake and output.
  2. Offer fluids during hands on care, snack time and with meals.
  3. Reports abnormal variances in intake to nurse.

TREATMENTS AND PROCEDURES

  1. Demonstrates ability to monitor vital signs accurately.
  2. Can accurately weigh residents using both bed scales and wheel chair scales.
  3. Can collect urine, stool and sputum specimens using proper technique.
  4. Can properly care for residents with ostomy.
  5. Can properly care for residents with indwelling catheters and suprapubic catheters.
  6. Can properly perform ROM on resident and can safely transfer, ambulate and reposition residents using proper body mechanics.

ASSESSMENT AND OBSERVATION

  1. Reports changes in residents condition/pertinent information to charge nurse or RN.
  2. Reports abnormalities in vital signs to charge nurse or RN as needed.
  3. Demonstrates the knowledge and skills necessary to provide care based on physical, psychosocial, communication, safety, education level and related criteria for resident per age specific needs.
  4. Adheres to all areas of confidentiality and resident rights policies.

DOCUMENTATION

  1. Demonstrates ability to accurately document on

C N A flow sheets and complete all resident logs on a daily bases.

COMMUNICATION

  1. Serves as resident advocate in providing information to appropriate staff as relayed from resident and/or family members.
  2. Maintains confidentiality and adheres to all aspects of the resident rights policy.
  3. Communicates effectively and professionally with residents, family, visitors and other departments to assist in meeting residents needs.

PATIENT TEACHING

  1. Assist LPN and RN in resident and family teaching by reinforcing information provided to them by nurse.

EQUIPMENT

Demonstrate the ability to use and maintain the following equipment in a safe manner.

  1. Urinary drainage system
  2. Oxygen set up
  3. Ace wraps and TEDS
  4. Indwelling and Suprapubic catheter
  5. Personal Protective Equipment
  6. Intercom and call light system
  7. Mechanical Lit
  8. Bed scales
  9. Wheel chair scales
  10. Vital Sign machine
  11. Thermometer

CHARGES

  1. Captures all chargeable items appropriately.

SAFETY

  1. Demonstrates knowledge of and uses side rails appropriately.
  2. Demonstrates knowledge of and uses falls protocol appropriately.
  3. Demonstrate knowledge of and use call lights appropriately, keeping them in reach of resident at all times.
  4. Wears back brace while doing direct resident care.
  5. Demonstrates knowledge of and uses good body mechanics while doing direct resident care.
  6. Uses universal precautions.
  7. Obtained annual physical.

EDUCATION

Successfully completed Education Day.

Successfully completed Validation of Skills.

Maintains valid CPR card.

Completed Care Learning for present year.

Attends staff meetings and in services.

Other duties as assigned.

RESTORATIVE AIDE

  • Has knowledge of and adheres to all Standards of Practice according to the Restorative Program Policy and Procedures.
  • Completes Restorative Flow Sheets daily including documentation of refusals and reason why, also completes weekly charting.
  • Has knowledge of and demonstrates proper techniques in Range of Motion Exercises, Ambulation, Bowel and Bladder Training Programs and Restorative Feeding Programs

Qualifications


QUALIFICATIONS:

  1. Must currently be certified as Certified Nursing Assistant in the State of West Virginia.
  2. Must successfully complete 90 day orientation as a CNA with satisfactory 90 day performance evaluation.
  3. Maintain current CPR certification.
  4. Must be willing to comply with all mandatory educational trainings.
  5. Must successful complete a Criminal Background Check.
