Part-Time Nurse Practitioner (Home Assessments)

We are seeking a dedicated and compassionate Part-Time Nurse Practitioner to join our team, specializing in conducting in-home assessments for individuals requiring Home Help Michigan services. The primary responsibility will be to perform comprehensive need assessments focusing on Activities of Daily Living (ADLs), complex care service needs, diagnoses, and illnesses.

**Key Responsibilities:**

- Conduct thorough in-home assessments to evaluate clients' ADLs and complex care needs.

- Document and report findings accurately.

**Qualifications:**

- Active Nurse Practitioner license in Michigan.

- Approved Medicaid provider enrolled in CHAMPS.

- Strong clinical assessment and documentation skills.

- Excellent communication and interpersonal abilities.

- Reliable transportation for home visits.
