In this pivotal role, your primary responsibility will encompass conducting ivirtual wellness risk adjustment assessments specifically for the Medicare demographic. Your mission is to evaluate the overall health and well-being of Medicare beneficiaries, ensuring precise and comprehensive risk adjustment coding to promote superior value-based healthcare.

Responsibilities:

- Conduct annual wellness examinations and health assessments for individuals with chronic conditions.

- Provide educational opportunities for patient health.

- Assist in addressing quality care gaps, such as screenings and laboratory tests.

- Act as an enthusiastic collaborator, actively contributing to the enhancement of care delivery.

Qualifications:

- Possession of an unencumbered Nurse Practitioner (NP) License for the state of California.

- Prior experience in in-home risk assessments is advantageous, though not mandatory.

- A minimum of 3 years of patient care experience is required, with a preference for experience in primary care, adult/geriatric care, and proficiency in Electronic Medical Record (EMR) systems.

- Experience in HCC coding, Risk adjustment, and population health a plus.

- Experience working with the geriatric population.

- Comfort and proficiency in utilizing EMR systems for electronic documentation (e-ClinicalWorks experience is highly recommended).

- Bilingual proficiency in Spanish, Mandarin, or Cantonese is a valuable asset.
