Key Responsibilities:

• Patient Care Excellence: Provide high-quality, compassionate, and patient-centered care to individuals across various age groups. Be able to administer medications to patients.
• Assessment and Diagnosis: Conduct thorough assessments, diagnose medical conditions, and develop personalized treatment plans..
• Health Promotion: Promote health education and preventive care to empower patients in managing their well-being.
• Record Keeping: Maintain accurate and up-to-date patient records using electronic health records (EHR) systems.

Requirements:

• Valid Nurse Practitioner or APRN license in Connecticut and current certification.
• Proven experience as a Nurse Practitioner/APRN, preferably in primary care or nutrition.
• Strong interpersonal and communication skills.
• Ability to work collaboratively in a team-oriented environment.

Schedule:

• Flexible scheduling.
• Days between the hours 10-6 p.m.
