Job Summary:

The Registered Nurse (RN) will provide professional nursing care to patients in their homes, adhering to the policies and procedures of Emerald Total Care . The RN will perform clinical assessments, develop care plans, administer medications, and collaborate with other healthcare professionals to deliver exceptional patient care. Additionally, the RN will be responsible for recruiting, hiring, and supervising Licensed Vocational Nurses (LVNs), ensuring they are properly trained and integrated into the home health team. This role requires strong clinical skills, effective communication, and the ability to work independently. The position is on a contract basis with flexible hours, including on-call and as-needed shifts.

Key Responsibilities:

  • Conduct comprehensive patient assessments to determine healthcare needs and develop individualized care plans.
  • Administer medications and treatments as prescribed by the physician, ensuring compliance with medical protocols and guidelines.
  • Monitor patient progress and adjust care plans as necessary to achieve optimal health outcomes.
  • Provide education and support to patients and their families regarding health conditions, treatment plans, and home care techniques.
  • Collaborate with physicians, therapists, social workers, and other healthcare professionals to coordinate patient care and ensure a multidisciplinary approach.
  • Maintain accurate and timely documentation of patient care, assessments, and interventions in electronic health records (EHR).
  • Respond to medical emergencies and provide appropriate interventions within the scope of practice.
  • Ensure compliance with all federal, state, and local regulations and standards of care.
  • Participate in ongoing education and training to stay current with best practices in home health care.
  • Recruit, hire, and supervise Licensed Vocational Nurses (LVNs), providing orientation, training, and ongoing support to ensure high standards of care.
  • Evaluate LVN performance and provide feedback, coaching, and professional development opportunities.

Qualifications:

  • Current and valid Registered Nurse (RN) license in the state of Texas.
  • Bachelor of Science in Nursing (BSN) preferred, Associate Degree in Nursing (ADN) required.
  • Minimum of [2-3] years of nursing experience, preferably in home health, critical care, or medical-surgical settings.
  • Experience in recruiting, hiring, and supervising nursing staff is highly desirable.
  • Current CPR certification.
  • Proficiency in using electronic health records (EHR) and other healthcare technology.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to work independently and manage multiple priorities in a dynamic environment.
  • Valid driver’s license and reliable transportation to travel to patient homes.

  • Ability to perform tasks involving physical activity, which may include heavy lifting and extensive bending and standing.
  • Capacity to handle exposure to various environmental conditions in patient homes.
