Connections Recovery Services is Hiring a Crisis Case Manager (PRN,Part-Time) Near Fremont, OH

Position Title: PRN, Case Manager for Mobile Crisis Response Team

Department: MCRT

Reports To: Resource Coordinator for Mobile Crisis Response Team

Date Approved 5-22-24

Is a member of the Mobile Crisis Response Team and responds to the location of a person in a mental health crisis. Responsible for providing case-management services while collaborating with law enforcement and mental health providers. Provides crisis support and stabilization activities including facilitating hospitalizations, conducts case follow-up, assists in designing safety plans, and coordinates with treatment teams and afterhours services. Works collaboratively with a Safety Officer.

ESSENTIAL DUTIES AND RESPONSIBILIITES

This job description is not intended to be all-inclusive. Employee will also perform other reasonably related duties assigned by immediate supervisor.

  • Provides emergency mental health support, and crisis interventions.
  • Collaborates with law enforcement and mental health providers while maintaining positive relationships.
  • Provides information and referrals to individuals that are in crisis, pre-crisis, or post-crisis.
  • Conducts follow-up visits at the direction of a Resource Coordinator or other mental health providers.
  • Creates safety plans for clients at risk.
  • Attends meetings at police departments or other mental health providers as needed.
  • Provides court advocacy to clients involved in the criminal justice system.
  • Provides support and crisis intervention to families of those with mental illness.
  • Completes all required charting, casework, documentation, and computer entry, on a timely basis.
  • Attends and participate in all required training, staff meetings, and case conferences.
  • Maintains a working knowledge of the mental health and human services systems in our region to provide appropriate referral and linkage for clients served.
  • Works in partnership with the Safety Officer to de-escalate persons in crisis, arrange suitable emergency hospitalization and transportation, provide support and conduct follow up visits.
  • Transports clients to appointments, court hearings, and other engagements with the intent of maintaining stabilization.
  • Performs tasks that provide for patients’ safety including ongoing screening and documentation of risk factors.
  • Maintain a valid Ohio driver's license.

EDUCATION/EXPERIENCE/SKILLS

•Bachelor’s degree in social work, psychology, or other human services related field required; master’s degree preferred.

•Experience in community behavioral health or crisis intervention setting is preferred.

•Holds a valid Ohio driver’s license and passes a criminal background check.

This position supervises the following positions:
