About being a Community Specialist. at Easterseals Redwood:
The Community Specialsit creates individualized (1:1) or small group community-based services and supports, based on individual’s goals, choices, and priorities as identified on each participants Individual Support Plan. Mentors and facilitates independence and integration into the community by strengthening personal competencies, social interaction, and community connectedness.
  • Remains knowledgeable in and supports development of Individual Support Plan (ISP) in collaboration with the individual, family, director, support coordinating agencies, and funders.
  • Provide community living/community integration supports, such as routine household tasks and maintenance, activities of daily living related to community integration, personal hygiene, shopping, money management, medication management, socialization, relationship building, leisure choices, or participation in community activities.
  • Provides or coordinates transportation to all activities.
  • Provides direct supervision, training, care, and positive interaction with adults on a one-on-one or small group basis.
  • Sets up and helps individuals use assistive/ adaptive technology including communication systems (with he help of the ATRC)
  • Uses an integrated teaching approach that embeds opportunities to improve social, communication, cognitive, and motor skills (developmental functions) during community integration routines and activities.
  • Adapts learning activities to support diverse abilities and learning styles. Provides assistance (supports participation) and offers/respects choices.
  • Engages individuals in physical activity relative to community experience to build strength/endurance/range of motion
  • Shapes development of appropriate behavior and social interaction skills by encouraging communication and positive interactions with others, modeling positive social skills, redirecting, and purposefully mentoring development of personal/social responsibility.
  • Provides opportunities for reflection after the community activity to support active learning.
  • Prepares/completes required documentation including time/attendance records; contact notes for each date of service to include supports and services provided; a detailed monthly summary note documenting progress in achievement of individual objectives; incident reports, etc. with accuracy and in a timely manner.
  • Provides care services required during the provision of community-based services and supports to address basic life needs including physical handling of individuals wheelchairs for transfers, positioning, personal hygiene, toileting, dressing, emergency response, and other tasks as needed.
  • Provides direct supervision and positive interaction with individuals during all personal care services.
  • Assists individuals with any type of personal care needed, while encouraging individuals to actively participate in all personal care activities to foster increasing levels of independence in addressing personal care needs.
  • Participates in medication administration training, as needed, and as directed. Administers medication, as required by policy and accepted practices.
  • Seeks medical attention for individuals exhibiting signs of Illness or injury. Administers first aid or CPR as need.
  • Communicates pertinent information regarding each individual’s services, care, and well-being to the assistant director.
What Easterseals Redwood needs from our Community Specialist:
  • bachelor’s degree in any field from an accredited college or university with one year of experience in the field of intellectual or developmental disabilities.
  • Highschool diploma and five years’ experience in the field of intellectual developmental or developmental disabilities as an alternative to a degree in the human service education requirement
