Job Description Summary :

Job Description :

The Community-Based Services (CBS) Program Coordinator supports individuals who are HIV positive, at high risk for HIV, and or particularly vulnerable to poor health outcomes, including those who are hidden or hard to reach.

The primary goal of the CBS Programs is to remove barriers to accessing and staying engaged in healthcare and supportive services.

Key qualities for this role include strong interpersonal skills, professionalism, exceptional communication with diverse populations, proficiency in project management, and a collaborative team-oriented approach.

The CBS Program Coordinator will help to build an overall culture of justice and equity while working to meet the goals of several different grant-funded programs.

The CBS Program Coordinator is expected to uphold company policies and procedures, ensuring that all interactions demonstrate high levels of professionalism and support the mission and policies of Community Health Center, Inc.

ROLE AND RESPONSIBILITIES

Program Development :

  • The CBS Program Coordinator works closely with the Community-Based Services Program Manager to establish the program’s direction, oversee its development and execution, and assist with effective resource allocations.
  • Identifies community needs through assessments, surveys, patient feedback, and collaboration with key stakeholders.
  • Coordinates the development of the program objectives, strategies, and activities to align with organizational goals, grant objectives, and community needs.
  • Ensure efficient program implementation by developing timelines, and work plans in collaboration with staff.

Community Engagement :

  • Build and maintain relationships with community members, leaders, and organizations.
  • Facilitate and participate in meetings and events to gather input, share information, and promote CBS programs.
  • Collaborate with community partners to leverage resources and support CBS program goals while ensuring there is no duplication of services.

Program Implementation :

  • Train and coach CBS staff to carry out the functions of the programs .
  • Supports team members by modeling standardization of care and enhancing clinical integration to optimize clinical effectiveness that improves patient experience, according to best practices.
  • Facilitate the on-going Implementation of the CKP Consumer Advisory Board (CAB) to ensure alignment with grant requirements to have an active CAB to provide feedback for the direction of program elements.

This includes the execution of membership, priorities, communication, training, and activities.

  • Collaborate with the CBS team to identify, prioritize, measure, and track social determinants of health and set goals and strategies for success.
  • Monitor, update, and address telephone encounters for CBS staff in electronic health records and ensure that they accurately reflect the status of each meeting with the consumer.
  • Ensure compliance with relevant regulations, policies, and funding requirements.
  • Actively participate in staff meetings, interdisciplinary team meetings, training, and supervisory sessions as required by the direct supervisor.

Communication and Reporting :

  • Communicate program goals, activities, and impact to stakeholders through reports, presentations, and outreach.
  • Maintain accurate records of program activities.
  • Provide updated information to all CKP team members about CBS services, including outcome measures, evaluation, and quality improvement needs.
  • Special assignments, projects, and other duties as assigned by direct supervisor.

QUALIFICATIONS

Educational Requirements :

Bachelor’s degree, preferably in Human Services or a related field preferred.

Experience :

  • Previous work experience in healthcare settings strongly preferred.
  • Demonstrated experience with community resource coordination, crisis intervention, service coordination, advocacy, and community outreach.
  • Familiarity with Federally Qualified Health Centers (FQHC) and HIV / AIDS related services, including Ryan White services, is an advantage.

Skills and Abilities :

  • Strong maturity, flexibility, and compassion in handling complex and sensitive situations.
  • Ability to effectively engage with diverse and vulnerable populations.
  • Outstanding communication skills, both oral and written, complemented by superb interpersonal and organizational skills.

Licensing and Mobility :

  • Must possess a valid driver’s license. Having a reliable vehicle is preferred.
  • Willingness and ability to travel from between sites as requested by supervisor.

Regulatory Knowledge :

Good understanding of HIPAA and other relevant state and federal regulations affecting healthcare practices.

PHYSICAL REQUIREMENTS / WORK ENVIRONMENT

At times, staff will enter tenement buildings and are required to climb up and downstairs. Most of the time, staff is expected to be in the office or community agencies doing collateral work with or on behalf of the consumer.

Travel time by foot or automobile is required.
